arch_uprobe_pre_xol needs to emulate a branch if a branch instruction
has been replaced with a breakpoint, but in fact an uninitialised local
variable was passed to the emulator routine instead of the original

Generic kernel code implements a weak version of set_orig_insn that
moves cached 'insn' from arch_uprobe to the original code location when
the trap is removed.
MIPS variant used arch_uprobe->orig_inst which was never initialised
properly, so this code only inserted a nop instead of the original
instruction. With that change orig_inst can also be safely removed.

cpu_has_fpu macro uses smp_processor_id() and is currently executed
with preemption enabled, that triggers the warning at runtime.
It is assumed throughout the kernel that if any CPU has an FPU, then all
CPUs would have an FPU as well, so it is safe to perform the check with
preemption enabled - change the code to use raw_ variant of the check to
avoid the warning.

The page structures associated with the vDSO pages in the kernel image
are calculated using virt_to_page(), which uses __pa() under the hood to
find the pfn associated with the virtual address. The vDSO data pointers
however point to kernel symbols, so __pa_symbol() should really be used
instead.
Since there is no equivalent to virt_to_page() which uses __pa_symbol(),
fix init_vdso_image() to work directly with pfns, calculated with
__phys_to_pfn(__pa_symbol(...)).
This issue broke the Malta Enhanced Virtual Addressing (EVA)
configuration which has a non-default implementation of __pa_symbol().
This is because it uses a physical alias so that the kernel executes
from KSeg0 (VA 0x80000000 -> PA 0x00000000), while RAM is provided to
the kernel in the KUSeg range (VA 0x00000000 -> PA 0x80000000) which
uses the same underlying RAM.
Since there are no page structures associated with the low physical
address region, some arbitrary kernel memory would be interpreted as a
page structure for the vDSO pages and badness ensues.

This patch fixes the possibility of a deadlock when bringing up
secondary CPUs.
The deadlock occurs because the set_cpu_online() is called before
synchronise_count_slave(). This can cause a deadlock if the boot CPU,
having scheduled another thread, attempts to send an IPI to the
secondary CPU, which it sees has been marked online. The secondary is
blocked in synchronise_count_slave() waiting for the boot CPU to enter
synchronise_count_master(), but the boot cpu is blocked in
smp_call_function_many() waiting for the secondary to respond to it's
IPI request.
Fix this by marking the CPU online in cpu_callin_map and synchronising
counters before declaring the CPU online and calculating the maps for
IPIs.

In the mipsr2_decoder() function, used to emulate pre-MIPSr6
instructions that were removed in MIPSr6, the init_fpu() function is
called if a removed pre-MIPSr6 floating point instruction is the first
floating point instruction used by the task. However, init_fpu()
performs varous actions that rely upon not being migrated. For example
in the most basic case it sets the coprocessor 0 Status.CU1 bit to
enable the FPU & then loads FP register context into the FPU registers.
If the task were to migrate during this time, it may end up attempting
to load FP register context on a different CPU where it hasn't set the
CU1 bit, leading to errors such as:

In microMIPS kernels, handle_signal() sets the isa16 mode bit in the
vdso address so that the sigreturn trampolines (which are offset from
the VDSO) get executed as microMIPS.
However commit ebb5e78cc634 ("MIPS: Initial implementation of a VDSO")
changed the offsets to come from the VDSO image, which already have the
isa16 mode bit set correctly since they're extracted from the VDSO
shared library symbol table.
Drop the isa16 mode bit handling from handle_signal() to fix sigreturn
for cores which support both microMIPS and normal MIPS. This doesn't fix
microMIPS only cores, since the VDSO is still built for normal MIPS, but
thats a separate problem.

Correct the cases missed with commit 9b26616c8d9d ("MIPS: Respect the
ISA level in FCSR handling") and prevent writes to read-only FCSR bits
there.
This in particular applies to FP context initialisation where any IEEE
754-2008 bits preset by `mips_set_personality_nan' are cleared before
the relevant ptrace(2) call takes effect and the PTRACE_POKEUSR request
addressing FPC_CSR where no masking of read-only FCSR bits is done.
Remove the FCSR clearing from FP context initialisation then and unify
PTRACE_POKEUSR/FPC_CSR and PTRACE_SETFPREGS handling, by factoring out
code from `ptrace_setfpregs' and calling it from both places.
This mostly matters to soft float configurations where the emulator can
be switched this way to a mode which should not be accessible and cannot
be set with the CTC1 instruction. With hard float configurations any
effect is transient anyway as read-only bits will retain their values at
the time the FP context is restored.

Fix a floating-point context restoration regression introduced with
commit 9b26616c8d9d ("MIPS: Respect the ISA level in FCSR handling")
that causes a Floating Point exception and consequently a kernel oops
with hard float configurations when one or more FCSR Enable and their
corresponding Cause bits are set both at a time via a ptrace(2) call.
To do so reinstate Cause bit masking originally introduced with commit
b1442d39fac2 ("MIPS: Prevent user from setting FCSR cause bits") to
address this exact problem and then inadvertently removed from the
PTRACE_SETFPREGS request with the commit referred above.

Whilst a PR_SET_FP_MODE prctl is performed there are decisions made
based upon whether the task is executing on the current CPU. This may
change if we're preempted, so disable preemption to avoid such changes
for the lifetime of the mode switch.

If a kernel doesn't support MSA context (ie. CONFIG_CPU_HAS_MSA=n) then
it will only keep 64 bits per FP register in thread context, and the
calls to set_fpr64 in restore_msa_extcontext will overrun the end of the
FP register context into the FCSR & MSACSR values. GCC 6.x has become
smart enough to detect this & complain like so:

The only way to trigger this code to run would be for a program to set
up an artificial extended MSA context structure following a sigframe &
execute sigreturn. Whilst this doesn't allow a program to write to any
state that it couldn't already, it makes little sense to allow this
"restoration" of MSA context in a system that doesn't support MSA.
Fix this by killing a program with SIGSYS if it tries something as crazy
as "restoring" fake MSA context in this way, also fixing the build error
& allowing for most of restore_msa_extcontext to be optimised out of
kernels without support for MSA.

In revision 1.12 of the MSA specification, the copy_u.w instruction has
been removed for MIPS32 & the copy_u.d instruction has been removed for
MIPS64. Newer toolchains (eg. Codescape SDK essentials 2015.10) will
complain about this like so:
arch/mips/kernel/r4k_fpu.S:290: Error: opcode not supported on this
processor: mips32r2 (mips32r2) `copy_u.w $1,$w26[3]'
Since we always copy to the width of a GPR, simply use copy_s instead of
copy_u to fix this.

After commit 92923ca3aacef63c92d ("mm: meminit: only set page reserved
in the memblock region"), the MIPS hibernation is broken. Because pages
in nosave data section should be "reserved", but currently they aren't
set to "reserved" at initialization. This patch makes hibernation work
again.

Commit f51246efee2b ("MIPS: Get rid of finish_arch_switch().") moved the
__restore_watch() call from finish_arch_switch() (i.e. after resume()
returns) to before the resume() call in switch_to(). This results in
watchpoints only being restored when a task is descheduled, preventing
the watchpoints from being effective most of the time, except due to
chance before the watchpoints are lazily removed.
Fix the call sequence from switch_to() through to
mips_install_watch_registers() to pass the task_struct pointer of the
next task, instead of using current. This allows the watchpoints for the
next (non-current) task to be restored without reintroducing
finish_arch_switch().

When showing backtraces in response to traps, for example crashes and
address errors (usually unaligned accesses) when they are set in debugfs
to be reported, unwind_stack will be used if the PC was in the kernel
text address range. However since EVA it is possible for user and kernel
address ranges to overlap, and even without EVA userland can still
trigger an address error by jumping to a KSeg0 address.
Adjust the check to also ensure that it was running in kernel mode. I
don't believe any harm can come of this problem, since unwind_stack() is
sufficiently defensive, however it is only meant for unwinding kernel
code, so to be correct it should use the raw backtracing instead.

When unwinding through IRQs and exceptions, the unwinding only continues
if the PC is a kernel text address, however since EVA it is possible for
user and kernel address ranges to overlap, potentially allowing
unwinding to continue to user mode if the user PC happens to be in the
kernel text address range.
Adjust the check to also ensure that the register state from before the
exception is actually running in kernel mode, i.e. !user_mode(regs).
I don't believe any harm can come of this problem, since the PC is only
output, the stack pointer is checked to ensure it resides within the
task's stack page before it is dereferenced in search of the return
address, and the return address register is similarly only output (if
the PC is in a leaf function or the beginning of a non-leaf function).
However unwind_stack() is only meant for unwinding kernel code, so to be
correct the unwind should stop there.

Copying the content of an MSA vector from user memory may involve TLB
faults & mapping in pages. This will fail when preemption is disabled
due to an inability to acquire mmap_sem from do_page_fault, which meant
such vector loads to unmapped pages would always fail to be emulated.
Fix this by disabling preemption later only around the updating of
vector register state.
This change does however introduce a race between performing the load
into thread context & the thread being preempted, saving its current
live context & clobbering the loaded value. This should be a rare
occureence, so optimise for the fast path by simply repeating the load if
we are preempted.
Additionally if the copy failed then the failure path was taken with
preemption left disabled, leading to the kernel typically encountering
further issues around sleeping whilst atomic. The change to where
preemption is disabled avoids this issue.

When calculate_cpu_foreign_map() recalculates the cpu_foreign_map
cpumask it uses the local variable temp_foreign_map without initialising
it to zero. Since the calculation only ever sets bits in this cpumask
any existing bits at that memory location will remain set and find their
way into cpu_foreign_map too. This could potentially lead to cache
operations suboptimally doing smp calls to multiple VPEs in the same
core, even though the VPEs share primary caches.
Therefore initialise temp_foreign_map using cpumask_clear() before use.

Commit 977e043d5ea1 ("MIPS: kernel: cps-vec: Replace mips32r2 ISA level
with mips64r2") leads to .set mips64r2 directives being present in 32
bit (ie. CONFIG_32BIT=y) kernels. This is incorrect & leads to MIPS64
instructions being emitted by the assembler when expanding
pseudo-instructions. For example the "move" instruction can legitimately
be expanded to a "daddu". This causes problems when the kernel is run on
a MIPS32 CPU, as CONFIG_32BIT kernels of course often are...
Fix this by dropping the .set <ISA> directives entirely now that Kconfig
should be ensuring that kernels including this code are built with a
suitable -march= compiler flag.

__clear_user() (and clear_user() which uses it), always access the user
mode address space, which results in EVA store instructions when EVA is
enabled even if the current user address limit is KERNEL_DS.
Fix this by adding a new symbol __bzero_kernel for the normal kernel
address space bzero in EVA mode, and call that from __clear_user() if
eva_kernel_access().

While the 5KE processors have never been taped out, they exists though
a CP0.PRId and experimental RTLs or QEMU implementations. Add a case
entry in the idle code, as they can use the standard idle loop like the
5K processors.

For bootloaders that support booting only ELF kernels and load only ELF
segments to memory there is no easy way to supply DTB without kernel
recompilation. For that purpose, create a section called .appended_dtb
that can be later updated with board-specific DTB using binutils e.g. at
kernel installation time.

Similar to how arm allows using selecting between bootloader arguments,
dtb arguments and both, allow to select them on mips. But since we have
less control over the place of the dtb do not modify it but instead use
the boot_command_line for merging them.
The default is "use bootloader arguments" to keep the current behaviour
as default.

Add user-mode implementations of gettimeofday() and clock_gettime() to
the VDSO. This is currently usable with 2 clocksources: the CP0 count
register, which is accessible to user-mode via RDHWR on R2 and later
cores, or the MIPS Global Interrupt Controller (GIC) timer, which
provides a "user-mode visible" section containing a mirror of its
counter registers. This section must be mapped into user memory, which
is done below the VDSO data page.
When a supported clocksource is not in use, the VDSO functions will
return -ENOSYS, which causes libc to fall back on the standard syscall
path.
When support for neither of these clocksources is compiled into the
kernel at all, the VDSO still provides clock_gettime(), as the coarse
realtime/monotonic clocks can still be implemented. However,
gettimeofday() is not provided in this case as nothing can be done
without a suitable clocksource. This causes the symbol lookup to fail
in libc and it will then always use the standard syscall path.
This patch includes a workaround for a bug in QEMU which results in
RDHWR on the CP0 count register always returning a constant (incorrect)
value. A fix for this has been submitted, and the workaround can be
removed after the fix has been in stable releases for a reasonable
amount of time.
A simple performance test which calls gettimeofday() 1000 times in a
loop and calculates the average execution time gives the following
results on a Malta + I6400 (running at 20MHz):
- Syscall: ~31000 ns
- VDSO (GIC): ~15000 ns
- VDSO (CP0): ~9500 ns

Add an initial implementation of a proper (i.e. an ELF shared library)
VDSO. With this commit it does not export any symbols, it only replaces
the current signal return trampoline page. A later commit will add user
implementations of gettimeofday()/clock_gettime().
To support both new toolchains and old ones which don't generate ABI
flags section, we define its content manually and then use a tool
(genvdso) to patch up the section to have the correct name and type.
genvdso also extracts symbol offsets ({,rt_}sigreturn) needed by the
kernel, and generates a C file containing a "struct mips_vdso_image"
containing both the VDSO data and these offsets. This C file is
compiled into the kernel.
On 64-bit kernels we require a different VDSO for each supported ABI,
so we may build up to 3 different VDSOs. The VDSO to use is selected by
the mips_abi structure.
A kernel/user shared data page is created and mapped below the VDSO
image. This is currently empty, but will be used by the user time
function implementations which are added later.

Prior to release 6 of the MIPS architecture it has been implementation
dependent whether masked interrupts cause a wait instruction to return,
so the kernel has effectively had to maintain a whitelist of cores upon
which it is safe to use the r4k_wait_irqoff cpu_wait implementation.
With MIPSr6 this is no longer implementation dependent and
r4k_wait_irqoff can always be used.
Remove the existing I6400 case which will no longer ever be hit, and was
incorrect anyway since I6400 & r6 in general doesn't have the WII bit.

Ensure the update to which core the core-other GCR regions reflect has
taken place before any core-other GCRs are accessed by placing a memory
barrier (sync instruction) between the write to the core-other registers

Document that CPC core-other accesses must take place within the bounds
of the CM lock, and begin using the CM lock functions where we access
the GCRs of other cores. This is required because with CM3 the CPC began
using GCR_CL_OTHER instead of CPC_CL_OTHER.

Introduce mips_cm_lock_other & mips_cm_unlock_other, mirroring the
existing CPC equivalents, in order to lock access from the current core
to another via the core-other GCR region. This hasn't been required in
the past but with CM3 the CPC starts using GCR_CL_OTHER rather than
CPC_CL_OTHER and this will be required for safety.

When debugging core bringup it is useful to see the state of the CPC
sequencer, so output that value if the core hasn't started within a
reasonable amount of time (1 second). This avoids simply appearing to
the user to hang if a secondary core fails to start.

Rather than patching the start of mips_cps_core_entry to provide the
base address of the CM GCRs, simply read that base address from the cop0
CMGCRBase register, converting from the physical address to an uncached
virtual address.

Provide support for outputting early debug information, in the form of
various register values should an exception occur, during the early
bringup of secondary cores. This code requires an ns16550-compatible
UART accessible from the secondary core, and is written in assembly due
to the environment in which such early exceptions occur where way may
not have a stack, be coherent or even have initialised caches.

WAIT instruction:
Thread enters wait state. No instructions are executed until an
interrupt occurs. The processor's clocks are stopped if both threads
are in idle mode.
Description:
Execution of this instruction puts the thread into wait state, an idle
mode in which no instructions are fetched or executed. The thread remains
in wait state until an interrupt occurs that is not masked by the
interrupt mask field in the Status register. Then, if interrupts are
enabled by the IE bit in the Status register, the interrupt is serviced.
The ERET instruction returns to the instruction following the WAIT
instruction. If interrupts are disabled, the processor resumes executing
instructions with the next sequential instruction.
Programming notes:
The WAIT instruction should be executed while interrupts are disabled
by the IE bit in the Status register. This avoids a potential timing
hazard, which occurs if an interrupt is taken between testing the counter
and executing the WAIT instruction. In this hazard case, the interrupt
will have been completed before the WAIT instruction is executed, so
the processor will remain indefinitely in wait state until the next
interrupt.

If the kernel may make use of 64 bit addresses outside of the
compatibility address space then we need to set KX such that those
accesses can succeed. Do so for MIPS64 kernels.

Set the Status.BEV bit throughout the early startup of a secondary core
such that if an exception occurs the core branches to one of the
exception vector entries from cps-vec.S, rather than branching to
whatever is set in EBase.
