The .remove() callback for a platform driver returns an int which makes
many driver authors wrongly assume it's possible to do error handling by
returning an error code. However the value returned is ignored (apart from
emitting a warning) and this typically results in resource leaks. To improve
here there is a quest to make the remove callback return void. In the first
step of this quest all drivers are converted to .remove_new() which already
returns void. Eventually after all drivers are converted, .remove_new() is
renamed to .remove().
Trivially convert this driver from always returning zero in the remove
callback to the void returning variant.

Drivers are typically supposed to be quiet unless they are actually
probed, but for some reason, USB host controllers seem to be exempt from
this rule, and happily broadcast their existence into the kernel log at
boot even if the hardware in question is nowhere to be found.
Let's fix that, and remove these pr_info() calls.

We don't need dev_err() messages when platform_get_irq() fails now that
platform_get_irq() prints an error message itself when something goes
wrong. Let's remove these prints with a simple semantic patch.

Now that the SPDX tag is in all USB files, that identifies the license
in a specific and legally-defined manner. So the extra GPL text wording
can be removed as it is no longer needed at all.
This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text. And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.
No copyright headers or other non-license-description text was removed.

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.
Update the drivers/usb/ and include/linux/usb* files with the correct
SPDX license identifier based on the license text in the file itself.
The SPDX identifier is a legally binding shorthand, which can be used
instead of the full boiler plate text.

When an Atmel SoC is suspended with the backup mode, the USB bus will be
powered down. As this is expected, do not return an error to the driver
core when ehci_resume detects it.

The ehci-atmel driver uses #ifdef to check for CONFIG_PM, but then
uses SIMPLE_DEV_PM_OPS, which leaves the references out when
CONFIG_PM_SLEEP is not defined, so we get a warning with
PM=y && PM_SLEEP=n:

The EHCI IP only needs the UTMI/UPLL (uclk) and the peripheral (iclk)
clocks to work properly. Remove the useless system clock (fclk).
Avoid calling set_rate on the fixed rate UTMI/IPLL clock and remove
useless IS_ENABLED(CONFIG_COMMON_CLK) tests (all at91 platforms have been
moved to the CCF).
This patch also fixes a bug introduced by 3440ef1 (ARM: at91/dt: fix USB
high-speed clock to select UTMI), which was leaving the usb clock
uninitialized and preventing the OHCI driver from setting the usb clock
rate to 48MHz.
This bug was caused by several things:
1/ usb clock drivers set the CLK_SET_RATE_GATE flag, which means the rate
cannot be changed once the clock is prepared
2/ The EHCI driver was retrieving and preparing/enabling the uhpck
clock which was in turn preparing its parent clock (the usb clock),
thus preventing any rate change because of 1/

Individual controller driver has different requirement for wakeup
setting, so move it from core to itself. In order to align with
current etting the default wakeup setting is enabled (except for
chipidea host).
Pass compile test with below commands:

Pull DMA mask updates from Russell King:
"This series cleans up the handling of DMA masks in a lot of drivers,
fixing some bugs as we go.
Some of the more serious errors include:
- drivers which only set their coherent DMA mask if the attempt to
set the streaming mask fails.
- drivers which test for a NULL dma mask pointer, and then set the
dma mask pointer to a location in their module .data section -
which will cause problems if the module is reloaded.
To counter these, I have introduced two helper functions:
- dma_set_mask_and_coherent() takes care of setting both the
streaming and coherent masks at the same time, with the correct
error handling as specified by the API.
- dma_coerce_mask_and_coherent() which resolves the problem of
drivers forcefully setting DMA masks. This is more a marker for
future work to further clean these locations up - the code which
creates the devices really should be initialising these, but to fix
that in one go along with this change could potentially be very
disruptive.
The last thing this series does is prise away some of Linux's addition
to "DMA addresses are physical addresses and RAM always starts at
zero". We have ARM LPAE systems where all system memory is above 4GB
physical, hence having DMA masks interpreted by (eg) the block layers
as describing physical addresses in the range 0..DMAMASK fails on
these platforms. Santosh Shilimkar addresses this in this series; the
patches were copied to the appropriate people multiple times but were
ignored.
Fixing this also gets rid of some ARM weirdness in the setup of the
max*pfn variables, and brings ARM into line with every other Linux
architecture as far as those go"

The correct way for a driver to specify the coherent DMA mask is
not to directly access the field in the struct device, but to use
dma_set_coherent_mask(). Only arch and bus code should access this
member directly.
Convert all direct write accesses to using the correct API.

The AT91 PMC (Power Management Controller) provides a USB clock used by
the different USB controllers (ehci, ohci and udc).
The atmel-ehci driver must configure the usb clock rate to 48Mhz in order
to get a fully functionnal USB host controller.
This configuration was formely done in mach-at91/clock.c, but will be
bypassed when moving to common clk framework.
This patch adds support for usb clock retrieval and configuration only if
CCF is enabled (CONFIG_COMMON_CLK).

Many USB host drivers contain code such as:
if (!pdev->dev.dma_mask)
pdev->dev.dma_mask = &tegra_ehci_dma_mask;
... where tegra_ehci_dma_mask is a global. I suspect this code originated
in commit 4a53f4e "USB: ehci-tegra: add probing through device tree" and
was simply copied everywhere else.
This works fine when the code is built-in, but can cause a crash when the
code is in a module. The first module load sets up the dma_mask pointer,
but if the module is removed and re-inserted, the value is now non-NULL,
and hence is not updated to point at the new location, and hence points
at a stale location within the previous module load address, which in
turn causes a crash if the pointer is de-referenced.
The simplest way of solving this seems to be to copy the code from
ehci-platform.c, which uses the coherent_dma_mask as the target for the
dma_mask pointer.

Separate the Atmel host controller driver from ehci-hcd host code
so that it can be built as a separate driver module.
This work is part of enabling multi-platform kernels on ARM;
however, note that other changes are still needed before Atmel can be
booted with a multi-platform kernel. This is currently planned for
Linux-3.11.
With the infrastructure added by Alan Stern in patch 3e0232039
"USB: EHCI: prepare to make ehci-hcd a library module", we can
avoid this problem by turning a bus glue into a separate
module, as we do here for the Atmel bus glue.

Convert all uses of devm_request_and_ioremap() to the newly introduced
devm_ioremap_resource() which provides more consistent error handling.
devm_ioremap_resource() provides its own error messages so all explicit
error messages can be removed from the failure code paths.

This patch (as1623) removes the ehci_port_power() routine and all the
places that call it. There's no reason for ehci-hcd to change the
port power settings; the hub driver takes care of all that stuff.
There is one exception: When the controller is resumed from
hibernation or following a loss of power, the ports that are supposed
to be handed over to a companion controller must be powered on first.
Otherwise the handover won't work. This process is not visible to the
hub driver, so it has to be handled in ehci-hcd.

The various devm_ functions allocate memory that is released when a driver
detaches. This patch uses these functions for data that is allocated in
the probe function of a platform device and is only freed in the remove
function.

This patch (as1564c) converts the EHCI platform drivers to use the
central ehci_setup() routine for generic controller initialization
rather than each having its own idiosyncratic approach.
The major point of difficulty lies in ehci-pci's many vendor- and
device-specific workarounds. Some of them have to be applied before
calling ehci_setup() and some after, which necessitates a fair amount
of code motion. The other platform drivers require much smaller
changes.
One point not addressed by the patch is whether ports should be
powered on or off following initialization. The different drivers
appear to handle this pretty much at random. In fact it shouldn't
matter, because the hub driver turns on power to all ports when it
binds to the root hub. Straightening that out will be left for
another day.

The two first HC capability registers (CAPLENGTH and HCIVERSION)
are defined as one 8-bit and one 16-bit register. Most HC
implementations have selected to treat these registers as part
of a 32-bit register, giving the same layout for both big and
small endian systems.
This patch adds a new quirk, big_endian_capbase, to support
controllers with big endian register interfaces that treat
HCIVERSION and CAPLENGTH as individual registers.

Several of the EHCI glue drivers either predate or were merged in the
same timeframe as API changes at the USB core level, resulting in some
missing endpoint_reset and clear_tt_buffer_complete callbacks.
This fixes up all of ehci-atmel, mxc, w90x900, and xilinx-of to tie in
the new helpers, which brings them in line with everyone else.

Add host USB High speed driver for at91sam9g45 series.
The host driver is an EHCI with its companion OHCI. EHCI is
handled by the new ehci-atmel.c whereas the OHCI is always
handled by ohci-at91.c.
