All kernel users of blkdev_report_zones() as well as applications use
through ioctl(BLKZONEREPORT) expect to potentially get less zone
descriptors than requested. As such, the use of the internal report
zones command execution loop implemented by blk_report_zones() is
not necessary and can even be harmful to performance by causing the
execution of inefficient small zones report command to service the
reminder of a requested zone array.
This patch removes blk_report_zones(), simplifying the code. Also
remove a now incorrect comment in dm_blk_report_zones().

For ZBC and ZAC zoned devices, the scsi driver revalidation processing
implemented by sd_revalidate_disk() includes a call to
sd_zbc_read_zones() which executes a full disk zone report used to
check that all zones of the disk are the same size. This processing is
followed by a call to blk_revalidate_disk_zones(), used to initialize
the device request queue zone bitmaps (zone type and zone write lock
bitmaps). To do so, blk_revalidate_disk_zones() also executes a full
device zone report to obtain zone types. As a result, the entire
zoned block device revalidation process includes two full device zone
report.
By moving the zone size checks into blk_revalidate_disk_zones(), this
process can be optimized to a single full device zone report, leading to
shorter device scan and revalidation times. This patch implements this
optimization, reducing the original full device zone report implemented
in sd_zbc_check_zones() to a single, small, report zones command
execution to obtain the size of the first zone of the device. Checks
whether all zones of the device are the same size as the first zone
size are moved to the generic blk_check_zone() function called from
blk_revalidate_disk_zones().
This optimization also has the following benefits:
1) fewer memory allocations in the scsi layer during disk revalidation
as the potentailly large buffer for zone report execution is not
needed.
2) Implement zone checks in a generic manner, reducing the burden on
device driver which only need to obtain the zone size and check that
this size is a power of 2 number of LBAs. Any new type of zoned
block device will benefit from this.

Zoned block devices (ZBC and ZAC devices) allow an explicit control
over the condition (state) of zones. The operations allowed are:
* Open a zone: Transition to open condition to indicate that a zone will
actively be written
* Close a zone: Transition to closed condition to release the drive
resources used for writing to a zone
* Finish a zone: Transition an open or closed zone to the full
condition to prevent write operations
To enable this control for in-kernel zoned block device users, define
the new request operations REQ_OP_ZONE_OPEN, REQ_OP_ZONE_CLOSE
and REQ_OP_ZONE_FINISH as well as the generic function
blkdev_zone_mgmt() for submitting these operations on a range of zones.
This results in blkdev_reset_zones() removal and replacement with this
new zone magement function. Users of blkdev_reset_zones() (f2fs and
dm-zoned) are updated accordingly.

In scsi_mq_setup_tags(), cmd_size is calculated based on zero size for the
scatter-gather list in case the low level driver uses SG_NONE in its host
template.
cmd_size is passed on to the block layer for calculation of the request
size, and we've seen NULL pointer dereference errors from the block layer
in drivers where SG_NONE is used and a mq IO scheduler is active,
apparently as a consequence of this (see commit 68ab2d76e4be ("scsi:
cxlflash: Set sg_tablesize to 1 instead of SG_NONE"), and a recent patch by
Finn Thain converting the three m68k NFR5380 drivers to avoid setting
SG_NONE).
Try to avoid these errors by accounting for at least one sg list entry when
calculating cmd_size, regardless of whether the low level driver set a zero
sg_tablesize.
Tested on 030 m68k with the atari_scsi driver - setting sg_tablesize to
SG_NONE no longer results in a crash when loading this driver.

The ILLEGAL REQUEST/INVALID FIELD IN CDB error generated by an attempt to
reset a conventional zone does not apply to the reset write pointer command
with the ALL bit set, that is, to REQ_OP_ZONE_RESET_ALL requests. Fix
sd_zbc_complete() to be quiet only in the case of REQ_OP_ZONE_RESET,
excluding REQ_OP_ZONE_RESET_ALL.
Since REQ_OP_ZONE_RESET is the only request handled by sd_zbc_complete(),
also simplify the code using a simple if statement.

nvme_mpath_clear_ctrl_paths() iterates through
the ctrl->namespaces list while holding ctrl->scan_lock.
This does not seem to be the correct way of protecting
from concurrent list modification.
Specifically, nvme_scan_work() sorts ctrl->namespaces
AFTER unlocking scan_lock.

Fix:
After taking scan_lock in nvme_mpath_clear_ctrl_paths()
down_read(&ctrl->namespaces_rwsem) as well to make list traversal safe.
This will not cause deadlocks because taking scan_lock never happens
while holding the namespaces_rwsem.
Moreover, scan work downs namespaces_rwsem in the same order.
Alternative: sort ctrl->namespaces in nvme_scan_work()
while still holding the scan_lock.
This would leave nvme_mpath_clear_ctrl_paths() without correct protection
against ctrl->namespaces modification by anyone other than scan_work.

In case there are controllers that are not associated with any RDMA
device (e.g. during unsuccessful reconnection) and the user will unload
the module, these controllers will not be freed and will access already
freed memory. The same logic appears in other fabric drivers as well.

Instead of picking the sub-command handler to execute in a nested
switch statement introduce a landing functions that calls out
to the appropriate sub-command handler.
This will allow us to have a common place in the handler to check
the transfer length in a future patch.

It's not apprporiate for the transports to set the data_len
field of the request which is only used by the core.
In this case, just use a variable on the stack to store the
length of the sgl for comparison.

None of the other transports check data_len which is verified
in core code. The function should instead check that the sgl length
is non-zero.

Introduce the new helper function nvme_lba_to_sect() to convert a device
logical block number to a 512B sector number. Use this new helper in
obvious places, cleaning up the code.

nvme_cleanup_cmd should be called for each call to nvme_setup_cmd
(symmetrical functions). Move the call for nvme_cleanup_cmd to the common
core layer and call it during nvme_complete_rq for the good flow. For
error flow, each transport will call nvme_cleanup_cmd independently. Also
take care of a special case of path failure, where we call
nvme_complete_rq without doing nvme_setup_cmd.

Fix the status code of canceled requests initiated by the host according
to TP4028 (Status Code 0x371):
"Command Aborted By host: The command was aborted as a result of host
action (e.g., the host disconnected the Fabric connection)."
Also in a multipath environment, unless otherwise specified, errors of
this type (path related) should be retried using a different path, if
one is available.

Currently, if the loop device receives a WRITE_ZEROES request, it asks
the underlying filesystem to punch out the range. This behavior is
correct if unmapping is allowed. However, a NOUNMAP request means that
the caller doesn't want us to free the storage backing the range, so
punching out the range is incorrect behavior.
To satisfy a NOUNMAP | WRITE_ZEROES request, loop should ask the
underlying filesystem to FALLOC_FL_ZERO_RANGE, which is (according to
the fallocate documentation) required to ensure that the entire range is
backed by real storage, which suffices for our purposes.

The following scenario results in an IO hang:
1) ctrl completes a request with NVME_SC_ANA_TRANSITION.
NVME_NS_ANA_PENDING bit in ns->flags is set and ana_work is triggered.
2) ana_work: nvme_read_ana_log() tries to get the ANA log page from the ctrl.
This fails because ctrl disconnects.
Therefore nvme_update_ns_ana_state() is not called
and NVME_NS_ANA_PENDING bit in ns->flags is not cleared.
3) ctrl reconnects: nvme_mpath_init(ctrl,...) calls
nvme_read_ana_log(ctrl, groups_only=true).
However, nvme_update_ana_state() does not update namespaces
because nr_nsids = 0 (due to groups_only mode).
4) scan_work calls nvme_validate_ns() finds the ns and re-validates OK.
Result:
The ctrl is now live but NVME_NS_ANA_PENDING bit in ns->flags is still set.
Consequently ctrl will never be considered a viable path by __nvme_find_path().
IO will hang if ctrl is the only or the last path to the namespace.
More generally, while ctrl is reconnecting, its ANA state may change.
And because nvme_mpath_init() requests ANA log in groups_only mode,
these changes are not propagated to the existing ctrl namespaces.
This may result in a mal-function or an IO hang.
Solution:
nvme_mpath_init() will nvme_read_ana_log() with groups_only set to false.
This will not harm the new ctrl case (no namespaces present),
and will make sure the ANA state of namespaces gets updated after reconnect.
Note: Another option would be for nvme_mpath_init() to invoke
nvme_parse_ana_log(..., nvme_set_ns_ana_state) for each existing namespace.

In shutdown/reboot paths, the timer is not stopped:
qla2x00_shutdown
pci_device_shutdown
device_shutdown
kernel_restart_prepare
kernel_restart
sys_reboot
This causes lockups (on powerpc) when firmware config space access calls
are interrupted by smp_send_stop later in reboot.
