Pull mm updates from Andrew Morton:
"The usual shower of singleton fixes and minor series all over MM,
documented (hopefully adequately) in the respective changelogs.
Notable series include:
- Lucas Stach has provided some page-mapping cleanup/consolidation/
maintainability work in the series "mm/treewide: Remove pXd_huge()
API".
- In the series "Allow migrate on protnone reference with
MPOL_PREFERRED_MANY policy", Donet Tom has optimized mempolicy's
MPOL_PREFERRED_MANY mode, yielding almost doubled performance in
one test.
- In their series "Memory allocation profiling" Kent Overstreet and
Suren Baghdasaryan have contributed a means of determining (via
/proc/allocinfo) whereabouts in the kernel memory is being
allocated: number of calls and amount of memory.
- Matthew Wilcox has provided the series "Various significant MM
patches" which does a number of rather unrelated things, but in
largely similar code sites.
- In his series "mm: page_alloc: freelist migratetype hygiene"
Johannes Weiner has fixed the page allocator's handling of
migratetype requests, with resulting improvements in compaction
efficiency.
- In the series "make the hugetlb migration strategy consistent"
Baolin Wang has fixed a hugetlb migration issue, which should
improve hugetlb allocation reliability.
- Liu Shixin has hit an I/O meltdown caused by readahead in a
memory-tight memcg. Addressed in the series "Fix I/O high when
memory almost met memcg limit".
- In the series "mm/filemap: optimize folio adding and splitting"
Kairui Song has optimized pagecache insertion, yielding ~10%
performance improvement in one test.
- Baoquan He has cleaned up and consolidated the early zone
initialization code in the series "mm/mm_init.c: refactor
free_area_init_core()".
- Baoquan has also redone some MM initializatio code in the series
"mm/init: minor clean up and improvement".
- MM helper cleanups from Christoph Hellwig in his series "remove
follow_pfn".
- More cleanups from Matthew Wilcox in the series "Various
page->flags cleanups".
- Vlastimil Babka has contributed maintainability improvements in the
series "memcg_kmem hooks refactoring".
- More folio conversions and cleanups in Matthew Wilcox's series:

- Rick Edgecombe has developed some fixes to stack guard gaps in the
series "Cover a guard gap corner case".
- Jinjiang Tu has fixed KSM's behaviour after a fork+exec in the
series "mm/ksm: fix ksm exec support for prctl".
- Baolin Wang has implemented NUMA balancing for multi-size THPs.
This is a simple first-cut implementation for now. The series is
"support multi-size THP numa balancing".
- Cleanups to vma handling helper functions from Matthew Wilcox in
the series "Unify vma_address and vma_pgoff_address".
- Some selftests maintenance work from Dev Jain in the series
"selftests/mm: mremap_test: Optimizations and style fixes".
- Improvements to the swapping of multi-size THPs from Ryan Roberts

Fixes the initialization code so that migration between different
memory types works as intended.
- David Hildenbrand has improved follow_pte() and fixed an errant
driver in the series "mm: follow_pte() improvements and acrn

- David also did some cleanup work on large folio mapcounts in his
series "mm: mapcount for large folios + page_mapcount() cleanups".
- Folio conversions in KSM in Alex Shi's series "transfer page to
folio in KSM".
- Barry Song has added some sysfs stats for monitoring multi-size
THP's in the series "mm: add per-order mTHP alloc and swpout
counters".
- Some zswap cleanups from Yosry Ahmed in the series "zswap
same-filled and limit checking cleanups".
- Matthew Wilcox has been looking at buffer_head code and found the
documentation to be lacking. The series is "Improve buffer head
documentation".
- Multi-size THPs get more work, this time from Lance Yang. His
series "mm/madvise: enhance lazyfreeing with mTHP in madvise_free"
optimizes the freeing of these things.
- Kemeng Shi has added more userspace-visible writeback
instrumentation in the series "Improve visibility of writeback".
- Kemeng Shi then sent some maintenance work on top in the series
"Fix and cleanups to page-writeback".
- Matthew Wilcox reduces mmap_lock traffic in the anon vma code in
the series "Improve anon_vma scalability for anon VMAs". Intel's
test bot reported an improbable 3x improvement in one test.
- SeongJae Park adds some DAMON feature work in the series
"mm/damon: add a DAMOS filter type for page granularity access recheck"
"selftests/damon: add DAMOS quota goal test"

It's already confusing that ARM 2-level v.s. 3-level defines SECT bit
differently on pmd/puds. Always use a macro which is much clearer.

With LPAE enabled, privileged no-access cannot be enforced using CPU
domains as such feature is not available. This patch implements PAN
by disabling TTBR0 page table walks while in kernel mode.
The ARM architecture allows page table walks to be split between TTBR0
and TTBR1. With LPAE enabled, the split is defined by a combination of
TTBCR T0SZ and T1SZ bits. Currently, an LPAE-enabled kernel uses TTBR0
for user addresses and TTBR1 for kernel addresses with the VMSPLIT_2G
and VMSPLIT_3G configurations. The main advantage for the 3:1 split is
that TTBR1 is reduced to 2 levels, so potentially faster TLB refill
(though usually the first level entries are already cached in the TLB).
The PAN support on LPAE-enabled kernels uses TTBR0 when running in user
space or in kernel space during user access routines (TTBCR T0SZ and
T1SZ are both 0). When running user accesses are disabled in kernel
mode, TTBR0 page table walks are disabled by setting TTBCR.EPD0. TTBR1
is used for kernel accesses (including loadable modules; anything
covered by swapper_pg_dir) by reducing the TTBCR.T0SZ to the minimum
(2^(32-7) = 32MB). To avoid user accesses potentially hitting stale TLB
entries, the ASID is switched to 0 (reserved) by setting TTBCR.A1 and
using the ASID value in TTBR1. The difference from a non-PAN kernel is
that with the 3:1 memory split, TTBR1 always uses 3 levels of page
tables.
As part of the change we are using preprocessor elif definied() clauses
so balance these clauses by converting relevant precedingt ifdef
clauses to if defined() clauses.

These macros will be used in a subsequent patch.
At one point these were part of the ARM32 KVM but that is no
longer the case.
Since these macros are only relevant to LPAE kernel builds, they
are added to pgtable-3level-hwdef.h

The cachepolicy variable gets initialized using a masked pmd
value. So far, the pmd has been masked with flags valid for the
2-page table format, but the 3-page table format requires a
different mask. On LPAE, this lead to a wrong assumption of what
initial cache policy has been used. Later a check forces the
cache policy to writealloc and prints the following warning:

This patch introduces a new definition PMD_SECT_CACHE_MASK for
both page table formats which masks in all cache flags in both
cases.

Modern ARMv7-A/R cores optionally implement below new
hardware feature:
- PXN:
Privileged execute-never(PXN) is a security feature. PXN bit
determines whether the processor can execute software from
the region. This is effective solution against ret2usr attack.
On an implementation that does not include the LPAE, PXN is
optionally supported.
This patch set PXN bit on user page table for preventing
user code execution with privilege mode.

For LPAE, we have the following means for encoding writable or dirty
ptes:
L_PTE_DIRTY L_PTE_RDONLY
!pte_dirty && !pte_write 0 1
!pte_dirty && pte_write 0 1
pte_dirty && !pte_write 1 1
pte_dirty && pte_write 1 0
So we can't distinguish between writeable clean ptes and read only
ptes. This can cause problems with ptes being incorrectly flagged as
read only when they are writeable but not dirty.
This patch renumbers L_PTE_RDONLY from AP[2] to a software bit #58,
and adds additional logic to set AP[2] whenever the pte is read only
or not dirty. That way we can distinguish between clean writeable ptes
and read only ptes.
HugeTLB pages will use this new logic automatically.
We need to add some logic to Transparent HugePages to ensure that they
correctly interpret the revised pgprot permissions (L_PTE_RDONLY has
moved and no longer matches PMD_SECT_AP2). In the process of revising
THP, the names of the PMD software bits have been prefixed with L_ to
make them easier to distinguish from their hardware bit counterparts.

The patch adds support for THP (transparent huge pages) to LPAE
systems. When this feature is enabled, the kernel tries to map
anonymous pages as 2MB sections where possible.

This patch adds support for hugetlbfs based on the x86 implementation.
It allows mapping of 2MB sections (see Documentation/vm/hugetlbpage.txt
for usage). The 64K pages configuration is not supported (section size
is 512MB in this case).

This patch moves the TTBR1 offset calculation and the T1SZ calculation out
of the TTB setup assembly code. This should not affect functionality in
any way, but improves code readability as well as readability of subsequent
patches in this series.

Sets up KVM code to handle all exceptions taken to Hyp mode.
When the kernel is booted in Hyp mode, calling an hvc instruction with r0
pointing to the new vectors, the HVBAR is changed to the the vector pointers.
This allows subsystems (like KVM here) to execute code in Hyp-mode with the
MMU disabled.
We initialize other Hyp-mode registers and enables the MMU for Hyp-mode from
the id-mapped hyp initialization code. Afterwards, the HVBAR is changed to
point to KVM Hyp vectors used to catch guest faults and to switch to Hyp mode
to perform a world-switch into a KVM guest.
Also provides memory mapping code to map required code pages, data structures,
and I/O regions accessed in Hyp mode at the same virtual address as the host
kernel virtual addresses, but which conforms to the architectural requirements
for translations in Hyp mode. This interface is added in arch/arm/kvm/arm_mmu.c
and comprises:
- create_hyp_mappings(from, to);
- create_hyp_io_mappings(from, to, phys_addr);
- free_hyp_pmds();

Add a method (hyp_idmap_setup) to populate a hyp pgd with an
identity mapping of the code contained in the .hyp.idmap.text
section.
Offer a method to drop this identity mapping through
hyp_idmap_teardown.
Make all the above depend on CONFIG_ARM_VIRT_EXT and CONFIG_ARM_LPAE.

This patch introduces the pgtable-3level*.h files with definitions
specific to the LPAE page table format (3 levels of page tables).
Each table is 4KB and has 512 64-bit entries. An entry can point to a
40-bit physical address. The young, write and exec software bits share
the corresponding hardware bits (negated). Other software bits use spare
bits in the PTE.
The patch also changes some variable types from unsigned long or int to
pteval_t or pgprot_t.
