When sync reset now event handling fails on mlx5_pci_link_toggle() then
no reset was done. However, since mlx5_cmd_fast_teardown_hca() was
already done, the firmware function is closed and the driver is left
without firmware functionality.
Fix it by setting device error state and reopen the firmware resources.
Reopening is done by the thread that was called for devlink reload
fw_activate as it already holds the devlink lock.

Refactor fw reset code to have the unload driver part done on
mlx5_fw_reset_complete_reload(), so if it was called by the PF which
initiated the reload fw activate flow, the unload part will be handled
by the mlx5_devlink_reload_fw_activate() callback itself and not by the
reset event work.
This will be used by the downstream patch to invoke devlink reload
callbacks with devlink lock held.

In case fw sync reset is called in parallel to device removal, device
might stuck in the following deadlock:
CPU 0 CPU 1
----- -----
remove_one
uninit_one (locks intf_state_mutex)
mlx5_sync_reset_now_event()
work in fw_reset->wq.
mlx5_enter_error_state()
mutex_lock (intf_state_mutex)
cleanup_once
fw_reset_cleanup()
destroy_workqueue(fw_reset->wq)
Drain the fw_reset WQ, and make sure no new work is being queued, before
entering uninit_one().
The Drain is done before devlink_unregister() since fw_reset, in some
flows, is using devlink API devlink_remote_reload_actions_performed().

Currently, health recovery will reload driver to recover it from fatal
errors. During the driver's load process, it would wait for FW to set the
pre-init bit for up to 120 seconds, beyond this threshold it would abort
the load process. In some cases, such as a FW upgrade on the DPU, this
timeout period is insufficient, and the user has no way to recover the
host device.
To solve this issue, introduce a new FW pre-init timeout for health
recovery, which is set to 2 hours.
The timeout for devlink reload and probe will use the original one because
they are user triggered flows, and therefore should not have a
significantly long timeout, during which the user command would hang.

Double clear of reset requested state can lead to NULL pointer as it
will try to delete the timer twice. This can happen for example on a
race between abort from FW and pci error or reset. Avoid such case using
test_and_clear_bit() to verify only one time reset requested state clear
flow. Similarly use test_and_set_bit() to verify only one time reset
requested state set flow.

mlx5-next 2022-22-02
The following PR includes updates to mlx5-next branch:
Headlines:
==========
1) Jakub cleans up unused static inline functions
2) I did some low level firmware command interface return status changes to
provide the caller with full visibility on the error/status returned by
the Firmware.
3) Use the new command interface in RDMA DEVX usecases to avoid flooding
dmesg with some "expected" user error prone use cases.
4) Moshe also uses the new command interface to grab the specific error
code from MFRL register command to provide the exact error reason for
why SW reset couldn't perform internally in FW.
5) From Mark Bloch: Lag, drop packets in hardware when possible
In active-backup mode the inactive interface's packets are dropped by the
bond device. In switchdev where TC rules are offloaded to the FDB
this can lead to packets being hit in the FDB where without offload
they would have been dropped before reaching TC rules in the kernel.
Create a drop rule to make sure packets on inactive ports are dropped
before reaching the FDB.
Listen on NETDEV_CHANGEUPPER / NETDEV_CHANGEINFODATA events and record
the inactive state and offload accordingly.

mlx5_tout_ms() returns a u64, we can't directly divide it.
This is not a problem here, @timeout which is the value
that actually matters here is already a ulong, so this
implies storing return value of mlx5_tout_ms() on a ulong
should be fine.

On sync_reset_now event the driver does reload and PCI link toggle to
activate firmware upgrade reset. When the firmware sends this event it
syncs the event on all PFs, so all PFs will do PCI link toggle at once.
To do PCI link toggle, the driver ensures that no other device ID under
the same bridge by checking that all the PF functions under the same PCI
bridge have same device ID. If no other device it uses PCI bridge link

After getting this event the driver expects device reset, either it will
