The LLDD and the stack currently process FPINs received from the fabric,
but the stack is not aware of any action taken by the driver to alleviate
congestion. The current interface between the driver and the SCSI stack is
limited to passing the notification mainly for statistics and heuristics.
The reaction to an FPIN could be handled either by the driver or by the
stack (marginal path and failover). Amend the interface to indicate if
action on an FPIN has already been reacted to by the LLDDs or not. Add an
additional flag to fc_host_fpin_rcv() to indicate if the FPIN has been
acknowledged/reacted to by the driver.
Also added a new event code FCH_EVT_LINK_FPIN_ACK to notify to the user
that the event has been acknowledged/reacted by the LLDD driver

Commit 6600593cbd93 ("block: rename BLK_EH_NOT_HANDLED to BLK_EH_DONE")
made it impossible for .eh_timed_out() implementations to call
scsi_done() without causing a crash.
Restore support for SCSI timeout handlers to call scsi_done() as follows:
* Change all .eh_timed_out() handlers as follows:
- Change the return type into enum scsi_timeout_action.
- Change BLK_EH_RESET_TIMER into SCSI_EH_RESET_TIMER.
- Change BLK_EH_DONE into SCSI_EH_NOT_HANDLED.
* In scsi_timeout(), convert the SCSI_EH_* values into BLK_EH_* values.

In order to help the compiler reason about the destination buffer in struct
fc_nl_event, add a flexible array member for this purpose. However, since
the header is UAPI, it must not change size or layout, so a union is used.
The allocation size calculations are also corrected (it was potentially
allocating an extra 8 bytes), and the padding is zeroed to avoid leaking
kernel heap memory contents.
Detected at run-time by the recently added memcpy() bounds checking:

In the original FPIN commit, stats were incremented by the event_count.
Event_count is the minimum # of events that must occur before an FPIN is
sent. Thus, its not the actual number of events, and could be significantly
off (too low) as it doesn't reflect anything not reported. Rather than
attempt to count events, have the statistic count how many FPINS cross the
threshold and were reported.

Remove the double listed FC_FPORT_DELETING from the mask creation.
Commit 260f4aeddb48 ("scsi: scsi_transport_fc: return -EBUSY for deleted
vport") added VC_VPORT_DELETING to the flag masks. This is not necessary as
FC_FPORT_DEL is defined as VC_FPORT_DELETED | FC_FPORT_DELETING.

Add store capability to the rport port_state using sysfs under
fc_remote_ports/rport-*/port_state.
With this the user can move the port_state from Marginal->Online and
Online->Marginal.
- Marginal: This interface will set SCMD_NORETRIES_ABORT bit in
scmd->state for all the pending I/Os on the SCSI device associated with
target port.
- Online: This interface will clear SCMD_NORETRIES_ABORT bit in
scmd->state for all the pending I/Os on the SCSI device associated with
target port.
The following interface is provided to set the port state to Marginal and
Online respectively:

Add a new interface, fc_eh_should_retry_cmd(), which checks if the cmd
should be retried or not by checking the rport state. If the rport state is
marginal it returns false to make sure there won't be any retries on the
cmd.
Make the fc_remote_port_delete(), fc_user_scan_tgt(), and
fc_timeout_deleted_rport() functions handle the new rport state.

Pull SCSI updates from James Bottomley:
"This is mostly update of the usual drivers: qla2xxx, hpsa, lpfc, ufs,
mpt3sas, ibmvscsi, megaraid_sas, bnx2fc and hisi_sas as well as the
removal of the osst driver (I heard from Willem privately that he
would like the driver removed because all his test hardware has
failed). Plus number of minor changes, spelling fixes and other
trivia.
The big merge conflict this time around is the SPDX licence tags.
Following discussion on linux-next, we believe our version to be more
accurate than the one in the tree, so the resolution is to take our
version for all the SPDX conflicts"
Note on the SPDX license tag conversion conflicts: the SCSI tree had
done its own SPDX conversion, which in some cases conflicted with the
treewide ones done by Thomas & co.
In almost all cases, the conflicts were purely syntactic: the SCSI tree
used the old-style SPDX tags ("GPL-2.0" and "GPL-2.0+") while the
treewide conversion had used the new-style ones ("GPL-2.0-only" and
"GPL-2.0-or-later").
In these cases I picked the new-style one.
In a few cases, the SPDX conversion was actually different, though. As
explained by James above, and in more detail in a pre-pull-request
thread:
"The other problem is actually substantive: In the libsas code Luben
Tuikov originally specified gpl 2.0 only by dint of stating:
* This file is licensed under GPLv2.
In all the libsas files, but then muddied the water by quoting GPLv2
verbatim (which includes the or later than language). So for these
files Christoph did the conversion to v2 only SPDX tags and Thomas
converted to v2 or later tags"
So in those cases, where the spdx tag substantially mattered, I took the
SCSI tree conversion of it, but then also took the opportunity to turn
the old-style "GPL-2.0" into a new-style "GPL-2.0-only" tag.
Similarly, when there were whitespace differences or other differences
to the comments around the copyright notices, I took the version from
the SCSI tree as being the more specific conversion.
Finally, in the spdx conversions that had no conflicts (because the
treewide ones hadn't been done for those files), I just took the SCSI
tree version as-is, even if it was old-style. The old-style conversions
are perfectly valid, even if the "-only" and "-or-later" versions are
perhaps more descriptive.

Currently the FC-NVMe driver is leverating the SCSI FC transport class to
access the remote ports. Which means that all FC-NVMe remote ports will be
visible to the fc transport layer, but due to missing definitions the port
roles will always be 'unknown'. This patch adds the missing definitions to
the fc transport class to that the port roles are correctly displayed.

Add a new event type - an FPIN event.
Add a new routine, fc_host_fpin_rcv(), that lldd's call when an FPIN is
received. The routine processes the fpin payload. For now, the routine
only logs an FPIN event.

There are two routines generating transport events that do the same thing
with only a couple of values set differently.
Refactor so there's a single routine doing the netlink operations to send
the event. All the differences are passed as arguments. Export the symbol
so the generic routine can be called by llds.
Modify the existing two event routines to use the helper.

The get_seconds() helper returns an 'unsigned long' value, which can
overflow on 32-bit architectures. Since the interface we pass it into
already uses a 64-bit type, we can just use ktime_get_real_seconds()
instead.
While we generally prefer local timestamps in CLOCK_MONOTONIC format
(ktime_get_seconds), this keeps using the CLOCK_REALTIME version in order
to maintain compatibility with existing code.

blk_mq_complete_request can only be called for blk-mq drivers, but when
removing the BLK_EH_HANDLED return value, two legacy request timeout
methods incorrectly got switched to call blk_mq_complete_request.
Call __blk_complete_request instead to reinstance the previous behavior.
For that __blk_complete_request needs to be exported.

Bsg holding a reference to the parent device may result in a crash if a
bsg file handle is closed after the parent device driver has unloaded.
Holding a reference is not really needed: the parent device must exist
between bsg_register_queue and bsg_unregister_queue. Before the device
goes away the caller does blk_cleanup_queue so that all in-flight
requests to the device are gone and all new requests cannot pass beyond
the queue. The queue itself is a refcounted object and it will stay
alive with a bsg file.

By completing the request entirely in the driver we can remove the
BLK_EH_HANDLED return value and thus the split responsibility between the
driver and the block layer that has been causing trouble.

The BLK_EH_NOT_HANDLED implies nothing happen, but very often that
is not what is happening - instead the driver already completed the
command. Fix the symbolic name to reflect that a little better.

Coverity-scan recently found a possible NULL pointer dereference in
fc_block_scsi_eh() as starget_to_rport() either returns the rport for
the startget or NULL.
While it is rather unlikely to have fc_block_scsi_eh() called without an
rport associated it's a good idea to catch potential misuses of the API
gracefully.

During failover there is a small race window between fc_remote_port_add()
and fc_timeout_deleted_rport(); the latter drops the lock after setting the
port to NOTPRESENT, so if fc_remote_port_add() is called right at that time
it will fail to detect the existing rport and happily adding a new
structure, causing rports to get registered twice.

When an rport is found in the bindings array there is no guarantee that
it had been a target port, so we need to call fc_remote_port_rolechg()
here to ensure the scsi_target_id is set correctly. Otherwise the port
will never be scanned.

Scsi_cmnd is an unsuitable argument for eh_device_reset_handler(),
eh_target_reset_handler(), and eh_host_reset_handler() which do not have
the scope of one single SCSI command. These callbacks tend to use
fc_block_scsi_eh() requiring scsi_cmnd. In order to start decoupling
above eh callbacks from scsi_cmnd, introduce a new variant of the
function called fc_block_rport() taking an fc_rport as argument.
Refactor the old fc_block_scsi_eh() to simply delegate to
fc_block_rport().

When trying to delete a vport via 'vport_delete' sysfs attribute we
should be checking if the port is already in state VPORT_DELETING; if so
there's no need to do anything.

This patch allows scsi drivers that expose virturalized fibre channel
devices but that do not expose rports to successfully rescan the scsi
bus via echo "- - -" > /sys/class/scsi_host/hostX/scan. Drivers can
create a pseudo rport and indicate FC_PORT_ROLE_FCP_DUMMY_INITIATOR as
the rport's role in fc_rport_identifiers. This insures that a valid
scsi_target_id is assigned to the newly created rport and it can meet
the requirements of fc_user_scan_tgt calling scsi_scan_target.

Instead define the timeout behavior purely based on the host_template
eh_timed_out method and wire up the existing transport implementations
in the host templates. This also clears up the confusion that the
transport template method overrides the host template one, so some
drivers have to re-override the transport template one.

Now that all conversions are done, move the FibreChannel bsg code over
to the bsg library.
This patch is derived from work done by Mike Christie in 2011 [1] but
only the iscsi parts got merged back then.
