Since we need to change the implementation, stop exposing internals.
Provide kref_read() to read the current reference count; typically
used for debug messages.
Kills two anti-patterns:

Add a new function `comedi_buf_write_n_available()` to return the amount
of buffer space available for writing, including space already allocated
by `comedi_buf_write_alloc()` plus any unallocated space available.
This is currently just for internal use by the comedi core, so is not
exported.

Rename the local function `comedi_buf_write_n_available()` to
`comedi_buf_write_n_unalloc()`. It is the amount of unallocated space
available in the buffer that is available to be allocated for writing
and does not include the space that has already been allocated for
writing. This is unlike the exported function
`comedi_buf_read_n_available()` which includes the space available to be
allocated for reading plus the space already allocated for reading. The
new name breaks the unintentional naming symmetry (and also clears the
way for the old name to be reused for a new function).

Introduce a new member to comedi_async to count the number of scans completed.
This member is cleared by comedi_buf_reset() along with the other comedi_async
members. It is incremented in comedi_inc_scan_progress() when the end of scan
is detected.
This member will be used to clean up the scan counting in the comedi drivers.

This inline helper function has been replaced with comedi_bytes_per_sample().
The same commit (bf33eb4b4f57) introduced a couple other related helper
functions a manipulate the sample size.
Use the new helper functions to remove the use of 'bytes_per_sample()' and
remove it.

Introduce a generic method to write samples to the async buffer.
The number of samples is first checked against the number of samples that
would fill the async buffer. The size of each sample is determined using
the bytes_per_sample() helper. If all the samples will fit in the async
buffer they are written to the buffer using comedi_write_array_to_buffer().
This will allow converting all the comedi drivers to use a common method to
write data to the async buffer.
Since comedi_write_array_to_buffer() sets the COMEDI_CB_BLOCK event after
writing the data, those events can be removed from the drivers.
In addition, comedi_inc_scan_progress() will automatically detect the end of
scan and set the COMEDI_CB_EOS event. Those events can also be removed from
the drivers.

full samples
A number of drivers currently use comedi_buf_get() to read single samples
from the the async buffer. This works but the drivers have to handle the
COMEDI_CB_BLOCK and COMEDI_CB_EOS events. Converting those drivers to use
comedi_buf_read_samples() moves the handling of those events into the core.
Modify comedi_buf_read_samples() so that the async buffer is checked for
the number of full samples available. Use that to clamp the number of
samples that will be read. This makes sure that only full samples are
read from the async buffer making comedi_buf_read_samples() mimic the
action of comedi_buf_get() with the added benifit of handling the events.

from the async buffer using comedi_read_array_from_buffer().

to read data from the async buffer.

reading the data, those events can be removed from the drivers.

The "comedi_fc" module was originally written and copyrighted by Frank
Mori Hess, but the functionality has been migrated into the core
"comedi" module. Move the copyright notices over to the affected .c
files in the core comedi module.

The "comedi_fc" module contains a few functions useful to Comedi
drivers. Their functionality is being migrated to the core "comedi"
module and renamed to start with the prefix `comedi_`. As part of this
migration, move `cfc_read_array_from_buffer()` into the core comedi
module and rename it to `comedi_read_array_from_buffer()`. Change the
external declaration of `cfc_read_array_from_buffer()` into an inline
function that calls `comedi_read_array_from_buffer()`.

Since `cfc_write_array_to_buffer()` is just an inline function that
calls `comedi_write_array_to_buffer()`, replace calls to the former to
the latter in the "comedi_fc.h" header. This is part of the migration
of functionality from the "comedi_fc" module to the core "comedi"
module.

Local function `comedi_buf_munge()` is called by
`comedi_buf_write_free()` to modify (or "munge") the data copied from
a device into the buffer.
Currently, the function takes a pointer to a `struct comedi_async` and
gets a pointer to the comedi subdevice from the `subdevice` member.
Change it to take a pointer to a `struct comedi_subdevice` and get a
pointer to the "async" structure from the `async` member.
The main motivation for this is to elimate the `subdevice` member of
`struct comedi_async`.

To avoid the circular dependency, just try to get the lock in
`comedi_mmap()` instead of blocking. Since the comedi device's main mutex
is heavily used, do a down-read of its `attach_lock` rwsemaphore
instead. Trying to down-read `attach_lock` should only fail if
some task has down-write locked it, and that is only done while the
comedi device is being attached to or detached from a low-level hardware
device.
Unfortunately, acquiring the `attach_lock` doesn't prevent another
task replacing the comedi data buffer we are trying to mmap. The
details of the buffer are held in a `struct comedi_buf_map` and pointed
to by `s->async->buf_map` where `s` is the comedi subdevice whose buffer
we are trying to map. The `struct comedi_buf_map` is already reference
counted with a `struct kref`, so we can stop it being freed prematurely.
Modify `comedi_mmap()` to call new function
`comedi_buf_map_from_subdev_get()` to read the subdevice's current
buffer map pointer and increment its reference instead of accessing
`async->buf_map` directly. Call `comedi_buf_map_put()` to decrement the
reference once the buffer map structure has been dealt with. (Note that
`comedi_buf_map_put()` does nothing if passed a NULL pointer.)
`comedi_buf_map_from_subdev_get()` checks the subdevice's buffer map
pointer has been set and the buffer map has been initialized enough for
`comedi_mmap()` to deal with it (specifically, check the `n_pages`
member has been set to a non-zero value). If all is well, the buffer
map's reference is incremented and a pointer to it is returned. The
comedi subdevice's spin-lock is used to protect the checks. Also use
the spin-lock in `__comedi_buf_alloc()` and `__comedi_buf_free()` to
protect changes to the subdevice's buffer map structure pointer and the
buffer map structure's `n_pages` member. (This checking of `n_pages` is
a bit clunky and I [Ian Abbott] plan to deal with it in the future.)

If a comedi device is automatically detached by `comedi_auto_unconfig()`
any data buffers associated with subdevices that support asynchronous
commands will be freed. If the buffer is mmapped at the time, bad
things are likely to happen! Prevent this by moving some of the buffer
details from `struct comedi_async` into a new, dynamically allocated,
and kref-counted `struct comedi_buf_map`. This holds a list of pages, a
reference count, and enough information to free the pages. The new
member `buf_map` of `struct comedi_async` points to a `struct
comedi_buf_map` when the buffer size is non-zero.
Provide a new helper function `comedi_buf_is_mapped()` to check whether
an a buffer is mmapped. If it is mmapped, the buffer is not allowed to
be resized and the device is not allowed to be manually detached by the
`COMEDI_DEVCONFIG` ioctl. Provide helper functions
`comedi_buf_map_get()` and `comedi_buf_map_put()` to manipulate the
reference count of the `struct comedi_buf_map`, which will be freed
along with its contents via the 'release' callback of the `kref_put()`
call. The reference count is manipulated by the vma operations and the
mmap file operation.
Now, when the comedi device is automatically detached, the buffer will
be effectively freed by calling `comedi_buf_alloc()` with a new buffer
size of 0. That calls local function `__comedi_buf_free()` which calls
`comedi_buf_map_put()` on the `buf_map` member to free it. It won't
actually be freed until the final 'put'.

Sample values in comedi are generally represented as unsigned values.
`comedi_buf_put()` and `comedi_buf_get()` use a `short` to hold the
16-bit data value being transferred to or from the comedi buffer.
Change them to use `unsigned short` for consistency.

When allocating a buffer to support asynchronous comedi commands, if a
DMA coherent buffer was requested but `CONFIG_HAS_DMA` is undefined,
bail out of local helper function `__comedi_buf_alloc()` with an error
message.

The core "comedi" module and the "mite" helper module for NI PCI devices
both have calls to `dma_alloc_coherent()` and `dma_free_coherent()`.
Those functions are only available if `CONFIG_HAS_DMA` is defined.
Apart from the "mite" module, the functions are only called for comedi
drivers that set `s->async_dma_dir` (where `s` is a pointer to a `struct
comedi_subdevice`) to anything other than `DMA_NONE`.
Change local helper functions `__comedi_buf_alloc()` and
`__comedi_buf_free()` to only call `dma_alloc_coherent()` and
`dma_free_coherent()` if `CONFIG_HAS_DMA` is defined.
Change the "Kconfig" to make the following configuration options depend
on `HAS_DMA`:
`COMEDI_MITE` - builds the "mite" module.
`COMEDI_NI_6527` - selects `COMEDI_MITE`.
`COMEDI_NI_65XX` - selects `COMEDI_MITE`.
`COMEDI_NI_670X` - selects `COMEDI_MITE`.
`COMEDI_NI_LABPC_PCI` - selects `COMEDI_MITE`.
`COMEDI_NI_PCIDIO` - selects `COMEDI_MITE`.
`COMEDI_NI_TIOCMD` - selects `COMEDI_MITE`.
`COMEDI_NI_660X` - selects `COMEDI_NI_TIOCMD`,
sets `s->async_dma_dir`.
`COMEDI_NI_PCIMIO` - selects `COMEDI_NI_TIOCMD`,
